Mused after reading that testosterone drives a pain-resolving molecule, known as interleukin-10. Blood level testing found that after trauma, interleukin-10 was found in higher levels in men than in women. Testing hormones confirmed that testosterone causes these higher levels. Studies are now underway applying testosterone patches to help those with chronic pain.

WHAT IS “TECH NECK” AND HOW DO I TREAT IT?

Millennial neck with wrinkles, pigmentation, and bands caused by loose muscle
“Tech neck” in broad terms ranges from laxity of neck tissues to the horizontal lines in one’s neck. Obviously, this increases with age, but it is showing in younger patients due to looking down at a cell phone for hours each day. Americans spend an average of 5 hours and 16 minutes on their phones daily, with Gen Z logging 6 hours and 27 minutes. One ad for the Procter and Gamble product, Olay, shows a woman applying it to her neck with the text, “Tech Neck Got You Down? Give it a Lift.”
(Did You Know? American writer and playwright Nora Ephron stated what women have known for centuries. The neck is a “dead giveaway” of one’s age.)
WHAT CAN BE DONE?
The gamut ranges from moisturizers and sunscreens to a surgical “necklift”. Many superficial treatments, like moisturizers and some mild acids, treat dry skin or don’t penetrate deep enough to improve wrinkling. Medical skin care products like retin-A and more concentrated acids stimulate improved blood supply and collagen turnover to help fine lines and skin tone. Microneedling and radiofrequency microneedling are the next steps to improve collagen turnover and skin tone. Injectable fillers like Juvederm help to elevate the wrinkles and soften them. Chemical peels are not a good idea, as there is a high tendency to lose pigmentation in neck skin. It does not heal the same way as the face.
If there is more involved than skin wrinkles, such as loose skin, excess fat, or the show of bands, then a necklift is preferable. Sometimes referred to as “deep plane,” this surgical procedure involves two layers. First is correcting excess fat by either direct removal or liposuction, followed by tightening the muscle that causes bands in the neck. Separately, the skin is then tightened and removed behind the ears to hide the scars. I’m happy to discuss this with anyone who feels they may have this problem.

LA DOLCE VITA, JOIE DE VIVRE:
Baby boomers, those born between 1945 and 1965, are redefining “old age”. The first lot of them are turning 80 this year. This generation is much more youth-obsessed and health-conscious than prior generations. The 15 million Americans over the age of 80 are expected to double within 20 years. Those 80 and older can expect to live, on average, to 93. Boomers are better educated and wealthier, although huge inequities exist compared to previous generations. The bottom line is that healthcare and health span are concerns. Healthcare costs continue to escalate. Health span reflects the quality of life, not just how long someone lives. Living at home or at an assisted living facility will continue to be topics of discussion, not only amongst individuals but also families.
